Could a D-SNP Be an Option?

These pieces must line up. Eligibility for one program by itself is not enough.

1. Medicare eligibility

You generally need Medicare Part A and Part B to join a Medicare Advantage Special Needs Plan.

2. Medicaid eligibility

You must qualify for an AHCCCS eligibility category accepted by the particular D-SNP.

3. Local plan availability

You must live in the plan’s service area. Options and requirements vary by county, ZIP code and year.

You may have enrollment opportunities outside AEP.

People who have Medicare and Medicaid may qualify for Special Enrollment Periods. The timing and plans available depend on your current eligibility, coverage and whether an integrated D-SNP is available.

What a D-SNP Is Designed to Do

A D-SNP combines Medicare Advantage coverage with plan features intended for people who also receive Medicaid assistance.

Medicare coverage

D-SNPs provide Medicare Part A and Part B benefits through a Medicare Advantage plan and include Medicare drug coverage.

Medicare–Medicaid coordination

The plan may help coordinate Medicare services with your AHCCCS coverage. The level of integration differs by plan and enrollment arrangement.

Plan-specific support

Networks, care coordination, costs and supplemental benefits vary. We verify what the actual plan provides instead of assuming every D-SNP is alike.

Is AHCCCS the same as Medicaid?

Can I change plans outside AEP?
